Information Sheet

Organizations should use the Information Sheet to submit their employee population, project officer names, and other data.

For an Excel version of the Information Sheet, please right-click the link below and "Save Target As..." to your computer. You may then simply e-mail the completed spreadsheet to lew@doingwhatmatters.org.
  • DOD Organizations: Be sure to include your civil service GENERAL SCHEDULE (GS) employees and NATIONAL SECURITY PERSONNEL SYSTEM (NSPS) employees under the appropriate headings; include uniformed personnel under the MILITARY heading.
     
  • Federal Prison Camp and Environmental Protection Agency: Include your Civil Service employees under the General Schedule section. NOTE: Your Public Health Service employees should be reported under the “MILITARY” section.
     
  • All Other Federal Organizations: Include your Civil Service employees under the General Schedule section.
Please download and fill out the attached spreadsheet, return the completed file by email
